The Illinois Supreme Court requires electronic filing (e-filing). You may be free (exempt) from this requirement if you have a good reason. Use this form to tell the court that you qualify to file court forms by paper.
You can use this form if you do not have a lawyer and at least one of the following 3 statements is true.
- You do not have the Internet or a computer in your home; your only access is through a public terminal at a courthouse, library, or other location; this is a financial or other hardship;
- You have a disability as defined by the ADA that prevents you from e-filing; OR
- You have trouble reading, writing, or speaking in English.
