Effective January 1, 2018, the State of Illinois will issue new birth certificates for people that have undergone gender transition treatment. The following is needed to change the person's sex designation on a birth certificate:
- Signed and dated declaration from a licensed health care professional or licensed mental health professional who has treated or evaluated the person;
- The declaration must say that the person has undergone the appropriate treatment for gender transition, or that the person has an intersex condition; and
- Payment of a $15 fee.
