1. Fill out your forms
- Notice of Motion and Order. Use the date of your next status call. Chicago status calls are held at 2 P.M. and downstate calls are held at 9 A.M. Make sure you check the Hearing under Sect. 19(b) box.
- Fill out the Proof of Service information on the back of the form. Remember to fill out your employer’s name and address. You must also get your signature notarized.
- Fill out the Petition for an Immediate Hearing Under Section 19(b) of the Act form. Remember to answer all questions on the front and back sides of the form.
2. Mail your forms
Mail or deliver one copy of Notice of Motion and Order and one copy of the Petition for an Immediate Hearing Under Section 19(b) of the Act to your employer at least 15 days before your status call date. Your status call date is the date you filled in on your Notice of Motion and Order. If you do not notify your employer at least 15 days before your next status call, the arbitrator will not set your case for trial.
4. Keep copies for your records
Make sure you keep at least one copy of each form for yourself.
