This Easy Form helps you ask a court for a legal name change for a child. If you need to change multiple children's names, you can request all the name changes at the same time on your forms. You must be 18 or older to use these forms.
The Easy Form does not e-file your forms. You can download the forms and e-filing instructions at the end of the program.
Name change forms are also available in Spanish on the Illinois Courts website. Courts only accept English language forms. If you need help with court forms, contact Illinois Court Help at (833) 411-1121.

¿Puedo usar este programa?
These forms can be used to change the name of up to four minor children.
To use these forms, the following must be true:
- You must be 18 or older, and
- You must have lived in Illinois for at least three months.
Also, you must have one of the following relationships with the minor children:
- Be the parent with custody,
- Be a guardian with legal custody, or
- The children have lived in your home for three years and are recognized as your adopted children.

- The name, birth date, address, birthplace, and criminal history of the children, and
- The name and address of the other parents or people with physical custody of the children.
