This Easy Form helps you make forms to collect on a money judgment from a debtor's bank or employer. It creates a Citiation to Discover Assets.

The Easy Form does not e-file your forms. You can download forms and e-filing instructions at the end of the program.

Learn more about collecting a judgment.

This program will help you make your court forms. It will ask questions for you to answer. At the end of the program, you will get completed court forms and instructions. You can save and print them, or the program can email them to you.

Form completion time: 10 minutes to 15 minutes

You will not be able to save your work in the middle of the program.

This program will help you avoid mistakes when filling out the court forms. Your case could be delayed if there are errors on your forms. If you want to download a PDF version, visit the Illinois Courts Standardized State Forms website. Use Adobe Reader to fill in the PDF forms or print the forms to fill out by hand.

The button below takes you to another website to complete the guided interview. Learn more about Easy Forms.

The interview and forms are in English.

Can I use this program?

Use this program when you have a judgment for money and are trying to find out:

  • How much the debtor's employer pays the debtor, or
  • If the debtor's bank has information about the debtor's property or income.

These forms do not work if you are owed money on a judgment for child support or spousal maintenance.

What do I need first?

To complete this program, you will need to know the:

  • Case number,
  • Courthouse address if court hearing is in person,
  • Circuit clerk phone, website, and remote hearing instructions if court hearing is remote,
  • Name and address of the debtor,
  • Name of the bank or employer,
  • Address for registered agent of the bank, or the address for the debtor's employer,
  • Date of judgment or the date it has been renewed (revived),
  • Amount of judgment, and
  • Amount that remains unpaid.

What documents will this create?

This program prepares the following forms. The forms you get depend on your answers.

  • How to do a Citation to Discover Assets to Debtor's Bank (Bank Garnishment)
  • How to do a Citation to Discover Assets to Debtor's Employer (Wage Deduction/Garnishment)
  • Citation to Discover Assets to Debtor's Bank
  • Citation to Discover Assets to Debtor's Employer
