An order of support is used by the judge to set the amount of support and how often it is to be paid. It can be used for cases that involve child support, maintenance, or both.
Can I use this form?
To use this form, you must have already:
- Started a case for child support or maintenance (formerly alimony), or
- Sought modification of an existing support order through a request to change child support.
If this applies to you, you will give this form to the judge. It is how the judge will write down their decisions.
What do I need first?
To complete this form, you will need:
- Your name, address, phone number, and email,
- The amount of child support to be paid,
- The amount of maintenance to be paid (if any)
- Your case number,
- The names and birth dates of any children that will be covered by the Order of Support,
- The name, address, and phone number of your employer,
- The name, address and phone number of the other party, and
- The social security numbers of you and the other parent/party (if available).
