If you want to appeal a Board of Review decision about your unemployment insurance benefits, you will need to file a Complaint for Administrative Review, an Affidavit of Last Known Address, and a Summons. 

Check with your local clerk of court's office to see if they have a Summons for administrative review actions for unemployment. If they do, use your local court's form. If no form exists, you can use this Summons form.
