This Easy Form does not mail your letter. You can download the letter and instructions at the end of the program.
This program will help you make your letter. It will ask questions for you to answer. At the end of the program, you will get a completed letter and instructions. You can save and print them, or the program can email them to you.
Form completion time: 5 minutes to 10 minutes
You will not be able to save your work in the middle of the program.
The interview and forms are in English.

- You must have worked for an employer within the past year, and
- Your employer must have 5 or more employees, not including family members.

- The date you were hired.
