Can I use this form?
Use this form to get your marriage annulled for the following reasons:
- Your spouse cannot have sex,
- Fraud (your spouse misled you),
- You were underage,
- One of the spouses could not consent, or
- Your spouse is already married.
What do I need first?
To complete this form, you will need to know:
- Date, time, and place you were married,
- Date of separation,
- The name of your spouse’s prior spouse (if your spouse is already married), and
- How your spouse misled you (if fraud).
