Affidavit of military service Blank form
This form is a written statement confirmed by oath or affirmation required to be filed by the Petitioner
in a divorce case. In the affidavit , the Petitioner states whether their spouse is in active military service and why the Petitioner believes this to be true. A default divorce will not be granted without this affidavit being completed.Can I use this form?

What do I need first?
To complete this form, you'll need:
- Your divorce case information, and
- To verify whether the other party in your case is the US military service. You can find servicemember information on the Servicemembers Civil Relief Act website. You will need to create an account to access this information.
