Use this form to prove that you adopted a child. It must be certified by the court.

What do I need first?

To complete this form, you will need:

  • The date of birth, place of birth, and names of the biological parents of the child being adopted, and 
  • The date of birth, place of birth, and social security number of each parent adopting the child.
