This Easy Form helps you ask the court to expunge offenses from your juvenile record. "Expunging" means erasing the arrests and court records.

The Easy Form does not e-file your forms. You can download the forms and e-filing instructions at the end of the program.

Learn more about expunging a juvenile record.

This program will help you make your court forms. It will ask questions for you to answer. At the end of the program, you will get completed court forms and instructions. You can save and print them, or the program can email them to you.

Form completion time: 20 minutes to 30 minutes

You will not be able to save your work in the middle of the program.

This program will help you avoid mistakes when filling out the court forms. Your case could be delayed if there are errors on your forms. If you want to download a PDF version, visit the Illinois Courts Standardized State Forms website. Use Adobe Reader to fill in the PDF forms or print the forms to fill out by hand.

The button below takes you to another website to complete the guided interview. Learn more about Easy Forms.

The interview and forms are in English.

Can I use this program?

To use this program:

  • You must have juvenile arrest or court records in Illinois, and
  • You must not be registered on the Juvenile Sex Offense Registry.

This program can create forms to expunge juvenile records from one county at a time. If you have records in more than one county, you can use the program again for each county.

What do I need first?

To complete this program, you will need copies of your juvenile records that include:

  • The date of arrest,
  • The arresting agency (police department),
  • The case outcome you received,
  • The court information if you saw a judge,
  • The date the sentence was completed (if applicable), and
  • The State's Attorney in the arresting county.

What documents will this create?

This program produces the following documents:

  • Request to Expunge Juvenile Records
  • Notice of Filing for Juvenile Expungement
  • Additional Arresting Agencies (if needed)
  • Additional Juvenile Records (if needed)
  • Order to Expunge Juvenile Records
  • Proof of Service
