Form completion time: 10 minutes to 15 minutes

Can I use this program?
To use this program, you must be unable to pay the court fees in your appeal.
What do I need first?
To use this program, you will need to know:
- The public benefits that you get,
- Your income each month,
- Your income over the past 12 months,
- Your monthly expenses, and
- The value of your belongings.
You will also need to know information about the:
- Circuit court case, including case number, county, and judge, and
- Appellate court case for which you are requesting a fee waiver, including appellate district and who filed the appeal.

Please note your fee waiver does not need to be filed in-person, but can be e-filed, mailed, or filed through other means allowed by your local court.
