This Easy Form helps you ask a court for a legal name change for a child. If you need to change multiple children's names, you can request all the name changes at the same time on your forms. You must be 18 or older to use these forms.

The Easy Form does not e-file your forms. You can download the forms and e-filing instructions at the end of the program.

Name change forms are also available in Spanish on the Illinois Courts website. Courts only accept English language forms. If you need help with court forms, contact Illinois Court Help at 833-411-1121.

This program will help you make your court forms. It will ask questions for you to answer. At the end of the program, you will get completed court forms and instructions. You can save and print them, or the program can email them to you.

Form completion time: 20 minutes to 30 minutes

You will be able to save your work in the middle of the program.

This program will help you avoid mistakes when filling out the court forms. Your case could be delayed if there are errors on your forms. If you want to download a PDF version, visit the Illinois Courts Standardized State Forms website. Use Adobe Reader to fill in the PDF forms or print the forms to fill out by hand.

The interview and forms are in English.

Can I use this program?

These forms can be used to change the name of up to 4 minor children.

To use these forms, the following must be true:

  • You must be 18 or older, and
  • You must have lived in Illinois for at least 6 months.

Also, you must have one of the following relationships with the minor children:

  • Be the parent with custody,
  • Be a guardian with legal custody, or
  • The children have lived in your home for 3 years and are recognized as your adopted children.

What do I need first?

To use these forms, you will need:

  • The name, birth date, address, birthplace, and criminal history of the children, and
  • The name and address of the other parents or people with physical custody of the children.

  • Request for Name Change (Minor Name Change)
  • Request for Name Change - Child Information
  • Request for Name Change - Additional Parent
  • Additional Children Request for Name Change
  • Request for Name Change - Additional Criminal History
  • Notice of Court Date for Request for Name Change (Minor Name Change)
  • Publication Notice of Court Date for Request for Name Change (Minor Name Change)
  • Motion to Waive Notice and Publication (Request for Name Change)
  • Order on Motion to Waive Notice & Publication (Request for Name Change)
  • Order for Name Change (Minor Name Change)
