Use this form if you have been unreasonably denied visitation Time spent between a child and a person who is not the parent with one or more children, and you are not their parent. For example, a grandparent, step-parent, or sibling.

Can I use this form?

You must have one of the following relationships to the child or children:

  • Grandparent or great-grandparent,
  • Sibling or half-sibling, or
  • Step-parent.

You must have been unreasonably denied visitation with the child or children.

What do I need first?

You will need:

  • Child or children's names and dates of birth,
  • Parents' names, and
  • County where the child or children live now.
