Petition for educational expenses - Cook County Blank form
A form to ask the judge to make the other parent pay educational expenses for your child who is 18 or older for cases in Cook county. If your case is outside of Cook county, you can check that county's website for forms.
Can I use this form?
To use this form:
- You must be the parent of a child over 18, but less than 23, years old, and
- You want the other parent to help pay for the child's education beyond high school, or to pay for high school expenses if the child has already turned 19 and is still in high school.
What do I need first?
To complete this form, you may need information found on one of the following documents:
- A Judgment of Dissolution of Marriage (Divorce Judgment),
- An Order of Parentage, or
- An Order of Child Support.
When you file this form with the clerk, you will also need a Cook County Notice of Motion.
