Help ILAO open opportunities for justice

This Easy Form helps you make a letter asking for time off work to deal with problems caused by violence. The letter can also help you ask for time off work if a family or household member has been killed in a crime.

The Easy Form does not mail your letter. You can download the letter and instructions at the end of the program.

Learn more about taking time off from work because of domestic abuse, sexual violence, or any other crime of violence.

This program will help you make your letter. It will ask questions for you to answer. At the end of the program, you will get a completed letter and instructions. You can save and print them, or the program can email them to you.

Form completion time: 5 minutes to 10 minutes

You will not be able to save your work in the middle of the program.

The interview and forms are in English.

Can I use this program?

To use this program, you must:

  • Be seeking time off from work to deal with problems caused by violence, and
  • Work in Illinois or for an Illinois employer.

What do I need first?

To use this program, you will need to know:

  • The name and address of your employer, and
  • The dates you need to take off work.

What documents will this create?

This program creates the following forms.

  • Instructions
  • Letter to request time off work
