This form can be used by a parent or guardian to designate another person to make day-to-day child care decisions for a minor child. It can be used when a parent or guardian is no longer willing or able to take care of the child.
Can I use this form?
You must be a parent or guardian that has parental responsibilities of a minor to use this form.
Important: Signing this form does not give the standby guardian authority to act. A petition must also be filed in court. Learn more about Creating a form to designate a standby guardian.
What do I need first?
To use this form, you need the following information:
- Name and birthdate of each child,
- Name and current address of standby guardian,
- Name and current address of successor standby guardian, and
- Two witnesses.
