This Easy Form will help you make forms to ask the Illinois Department of Employment Security (IDES) to reconsider a decision to deny you unemployment benefits. It creates a Request for Reconsideration of Claims Adjudicator's Determination that asks for a Referee hearing.

The Easy Form does not file your forms. You can download the forms and instructions at the end of the program.

This program will help you make your forms. It will ask questions for you to answer. At the end of the program, you will get completed forms and instructions. You can save and print them, or the program can email them to you.

Form completion time: 5 minutes to 10 minutes

Can I use this program?

To use this program, you must have been denied unemployment benefits and you think that the decision against you is wrong.

Note: If you already had a Referee hearing, you should use the Unemployment benefits appeal after a hearing program instead.

What do I need first?

To complete this program, you will need the UI finding letter you received from IDES. This is also known as a determination letter. Sometimes the finding letter and determination letter are 2 different documents. 

You will also need your personal information and the reason for your appeal.
