This form is used to ask the Illinois Governor for executive clemency. You can complete this form electronically, but you cannot submit this form electronically to the Prison Review Board.
Can I use this form?
Any conviction in the State of Illinois is eligible for clemency, no matter what the charges were or when the conviction occurred. Historically, the longer it has been since your last conviction, the stronger the chance you have of receiving clemency.
What do I need first?
Depending on the type of clemency you request, required information will include, but may not be limited to, the following:
- Notarized signatures,
- Your name, address, date of birth, social security number, and state prisoner number,
- The information about your convictions including your offense, case number, date of arrest, and sentence,
- A personal life history, and
- Reasons for seeking clemency.
