Petition for Stalking No Contact Order - Cook County Blank form
This form opens a case to ask the court to order someone who is “stalking” you to stay away from you and/or stop contacting you. This form is only for Cook County.

Can I use this form?
To use these forms:
- The stalker can be someone who is a family member, current or ex-spouse, or current or ex-boyfriend or girlfriend. It can also be someone who does not have these relationships with you.
- If the stalker is a family member, current or ex-spouse, or current or ex-boyfriend of girlfriend, you should consider asking for an Order of Protection instead as it can give you more options.
- The stalker did something that made you fearful or caused emotional distress on at least two separate occasions (one incident is not enough), and
- By filing these forms, you are saying Cook county is the county where:
- The stalking took place,
- You live, or
- The stalker lives.
What do I need first?
To complete these forms, you will need:
- Either to provide your address or to provide an alternative address where you can receive court documents if you do not want the stalker to know the address where you are staying,
- The stalker's name, gender, and race, and
- Details about each incident of stalking including when it took place, where you were, and what happened.
