- Eviction,
- Foreclosure
- Small claims,
- Detinue,
- Administrative review,
- Order of protection, Civil no contact order, or Stalking no contact order

What do I need first?
To complete this form, you will need to know:
- The case number,
- The amount you are suing for (if any),
- The method used for delivering the summons, and
- The name and address of each defendant/respondent.
