, bank, or company that provided the thief with unauthorized credit, goods, or services. You can also include a copy of your police report if you have filed one.Note: Do not file this form in a court case. To use this form, you must have been a victim of identity theft.
Contact the creditor, bank, or company that provided the thief with unauthorized credit, goods, or services to find out:
- If they'll accept this form, and
- If the form needs to be notarized.
What do I need first?
You will need:
- Victim information,
- How the fraud occurred,
- Information on any law enforcement actions, and
- Information about accounts that were opened in your name, if any.
