If you were sued over unpaid debt due to identity theft:
- Complete an Identity theft affidavit for mailing to any creditor, bank, or company that provided the thief with unauthorized credit, goods, or services, and
- Fill out the Illinois Supreme Court form linked below to file in the court case.
Can I use this form?
You must be a victim of identity theft who was sued because of a collection related to the identity theft. You must have already filled out an Identity theft affidavit.
What do I need first?
You will need:
- Your name,
- Plaintiff's name,
- Case number, and
- How you delivered this form to other parties in your case.
