  1. Complete an Identity theft affidavit for mailing to any creditor, bank, or company that provided the thief with unauthorized credit, goods, or services, and
  2. Fill out the Illinois Supreme Court form linked below to file in the court case.

Can I use this form?

You must be a victim of identity theft who was sued because of a collection related to the identity theft. You must have already filled out an Identity theft affidavit.

What do I need first?

You will need:

  • Your name,
  • Plaintiff's name,
  • Case number, and
  • How you delivered this form to other parties in your case.
